Met een enthousiast team van 150 medewerkers ondersteunen we 300 jongeren en gezinnen waar de ontwikkelkansen of veiligheid ernstig bedreigd zijn. Wij maken samen met partners voor elke jongere en hun gezin een plan op maat.

Wij kiezen voor Signs of Safety als benaderingsmodel voor alle trajecten. In de residentiële teams zetten we sterk in op de ontwikkeling van waakzame zorg en nieuwe autoriteit.

WAT DOE JE ALS MIJN OUTREACH-BEGELEIDER?

· Gezinnen en het ruimere netwerk intensief ondersteunen in hun eigen leefomgeving (buiten de schooluren) op verschillende levensdomeinen, en dit vanuit een faciliterende houding

· Een vertrouwensband opbouwen met gezin en jongere.

· Meewerken aan betekenisvolle trajecten voor kinderen/jongeren (van 0 tot 25 jaar) en hun netwerk met focus op veiligheid en welzijn van de jongere en de krachten van iedere betrokkene

· Meehelpen de rode draad vast te houden in overleg met de trajectbegeleider.

· Samen met de trajectbegeleider het gezin ondersteunen in het uitbouwen van een netwerk en andere relevante partners betrekken en activeren.

· Actief aanwezig zijn op overleg- of schakelmomenten (netwerkoverleg, evolutiebespreking,....)

· Plan- en procesmatig werken met de nodige flexibiliteit op maat (zelforganiserend).

· Positief, oplossingsgericht en analytisch denken/werken.

· Open en duidelijk communiceren.

· Kritisch reflecteren op je eigen handelen en bijsturen.
