- ASVZ - Udenhout ( Noord-Brabant ) - Online sinds: vandaag ASVZ Voor Samenthuis in Udenhout zijn wij op zoek naar een persoonlijk begeleider. Op deze woning wonen zes cliënten met een ernstige verstandelijke en meervoudige beperking (EVMB). Zij hebben behoefte aan intensieve begeleiding, nabijheid en ondersteuning die aansluit bij hun mogelijkheden en belevingswereld. Wat ga je doen? Als persoonlijk begeleider ben je verantwoordelijk voor de begeleiding van een aantal cliënten. Je stelt samen met het team, ouders en verwanten het begeleidingsplan op, houdt dit actueel en zorgt ervoor dat de afspraken terugkomen in de dagelijkse ondersteuning. Je werkt actief mee op de woning en ondersteunt cliënten bij alle dagelijkse momenten. Persoonlijke verzorging is een belangrijk onderdeel van je werk. Je helpt cliënten bij onder andere wassen, aankleden, eten en drinken en zorgt ervoor dat zij zich prettig en comfortabel voelen. Omdat cliënten zichzelf niet altijd kunnen uiten, is het belangrijk dat je goed kijkt en luistert naar kleine signalen. Je leert cliënten goed kennen en ontdekt wat zij prettig vinden, waar zij behoefte aan hebben en hoe zij contact maken. Je biedt structuur, veiligheid en zorgt samen met het team voor waardevolle momenten in het dagelijks leven. Samenthuis is een ouderinitiatief. Ouders en verwanten zijn nauw betrokken bij de woning en het leven van de cliënten. Je werkt daarom intensief samen met familie en andere betrokkenen om de ondersteuning zo goed mogelijk aan te laten sluiten. Je: - stelt begeleidingsplannen op en evalueert deze - bewaakt de voortgang en kwaliteit van de begeleiding - ondersteunt cliënten bij persoonlijke verzorging en dagelijkse activiteiten - neemt deel aan overleggen met collega's, ouders, verwanten en andere disciplines - zorgt voor duidelijke afspraken binnen het team Je werkt onregelmatige diensten, waaronder ook slaapdiensten.
