Voor deze vacature zoeken wij een persoonlijk begeleider die het leuk vindt om veel te schakelen in een diversiteit aan werkzaamheden op een dag, alles gericht op de zorgvraag van de cliënt. In de functie van persoonlijk begeleider maak je de zorg- en ondersteuningsplannen en werk je mee aan de uitvoering daarvan. Ook coördineer je de activiteiten die uit de plannen voortvloeien. Je onderhoudt en voert regie over de externe contacten van de cliënt (bijvoorbeeld met instanties en verwanten). Daarnaast biedt je persoonlijke begeleiding bij dagelijkse verzorgingstaken (ADL), variërend van lichte ondersteuning tot volledige hulp. Je helpt cliënten structuur aan te brengen in hun dagritme en begeleidt hen bij huishoudelijke taken en vrijetijdsbesteding. Je ondersteunt jouw cliënten bij het behouden en versterken van hun plaats in onze samenleving. Hun keuzes en behoeftes zijn heel verschillend. De een kan het verlangen hebben om zijn of haar systeem uit te breiden, terwijl voor de ander het creëren van veiligheid binnen de eigen woning het belangrijkst is. Zo zijn er tal van vragen en situaties waarbij jij als creatieve professional de ruimte krijgt om hen zo goed mogelijk te ondersteunen. En als een cliënt zich onveilig of onbegrepen voelt, zorg jij voor rust, stabiliteit en betrouwbaarheid. Onze volwassen cliënten hebben een ontwikkelingsniveau van 1-3 jaar.
