Op woonlocatie Julianahof in Leidschendam zoeken we een Coördinerend Begeleider die rust brengt, overzicht houdt én dichtbij de bewoners staat. Wij zoeken iemand voor 24-32 uur in de week. Jouw rol Als Coördinerend Begeleider - binnen Philadelphia ook wel Persoonlijk Begeleider genoemd - ben jij de verbindende factor tussen cliënt, netwerk en team. Je kijkt verder dan alleen de dagelijkse begeleiding. Jij bewaakt het grotere geheel rondom de zorg en ondersteuning van jouw bewoners. Je werkt met volwassenen van 24 tot 80 jaar met een verstandelijke beperking (VB) of lichte verstandelijke beperking (LVB). Sommige bewoners hebben vooral behoefte aan begeleiding en structuur, anderen vragen daarnaast lichte ADL-zorg of extra nabijheid door ouderdomsklachten of veranderend gedrag. Wat jouw werk bijzonder maakt? Je staat niet langs de zijlijn te coördineren. Je werkt actief mee op de groep, kent de bewoners goed en merkt daardoor snel wat iemand nodig heeft. - ben je verantwoordelijk voor ondersteuningsplannen en het bewaken van doelen; - stem je af met verwanten, behandelaren, dagbesteding en collega's; - organiseer je MDO's en onderhoud je contact met wettelijke vertegenwoordigers; - bied je begeleiding en waar nodig lichte verzorging of ADL-ondersteuning; - creëer je rust, duidelijkheid en een veilige woonomgeving; - signaleer je veranderingen in gedrag of welzijn en handel je hierop; - werk je mee in dag-, avond-, weekend- en slaapdiensten. Je krijgt bij Julianahof veel zelfstandigheid. Dat vraagt iemand die stevig staat, overzicht houdt en verantwoordelijkheid durft te nemen.
