- ASVZ - Sliedrecht ( Zuid-Holland ) - Online sinds: vandaag ASVZ Op Schoorstraat 4-06 wonen zeven ouder wordende bewoners met een matige tot ernstige verstandelijke beperking. De zorgvragen zijn verschillend en lichamelijke ondersteuning speelt een belangrijke rol. Als persoonlijk begeleider kijk je samen met bewoners, hun netwerk en collega's hoe we hun kwaliteit van bestaan zo groot mogelijk maken. Daarbij houden we oog voor eigen regie, persoonlijke wensen en een fijne sfeer in huis. Wat ga je doen? Als persoonlijk begeleider ben je verantwoordelijk voor de coördinatie van de begeleiding van een aantal bewoners. Je verdiept je in hun levensverhaal, mogelijkheden, behoeften en ondersteuningsvragen. Vanuit die kennis stel je begeleidingsplannen op, houd je deze actueel en bespreek je de voortgang met bewoners, verwanten en collega's. Binnen deze woning vraagt de ouder wordende doelgroep om extra aandacht voor lichamelijk welzijn. Je houdt veranderingen in gezondheid, gedrag en dagelijkse mogelijkheden in de gaten en zorgt dat belangrijke signalen op de juiste plek terechtkomen. Je stemt de ondersteuning af met betrokken disciplines en bewaakt dat gemaakte afspraken worden nagekomen. Je werkt vanuit de gedachte dat bewoners zoveel mogelijk zelf de regie houden. Ook wanneer iemand veel begeleiding of verzorging nodig heeft, zoek je naar manieren om keuzes mogelijk te maken. Samen met de bewoner en het team kijk je hoe je iedere dag kunt bijdragen aan een betekenisvol en prettig leven. Je: - coördineert de begeleiding van een aantal bewoners - stelt begeleidingsplannen op, evalueert deze en houdt ze actueel - brengt wensen, mogelijkheden en ondersteuningsvragen van bewoners in kaart - bewaakt de voortgang en kwaliteit van de gemaakte afspraken - signaleert veranderingen in lichamelijk welzijn en gedrag - stemt af met verwanten, collega's en betrokken disciplines Jij herkent jezelf hierin Je bent een persoonlijk begeleide...
