Persoonlijk Begeleider Gehandicaptenzorg LVB/MVB | Brakel | 26 uur Bij Philadelphia heet deze functie officieel Coördinerend Begeleider. Wil jij verantwoordelijkheid nemen voor goede begeleiding én zelf dichtbij bewoners blijven? Op De Regenboog in Brakel werk je met volwassenen met een lichte of matige verstandelijke beperking. Geen bewoner is hetzelfde: de één heeft vooral behoefte aan coaching en structuur, terwijl een ander meer nabijheid of ondersteuning bij ADL nodig heeft. Voor deze bijzondere woonlocatie zoeken wij een Persoonlijk Begeleider voor 26 uur per week . Jouw rol Als Persoonlijk Begeleider combineer je de dagelijkse begeleiding van bewoners met de coördinatie van hun ondersteuning. Je staat dus niet op afstand plannen te maken. Juist doordat je meewerkt op de groep, weet je wat er speelt en kun je goed beoordelen wat een bewoner nodig heeft. Je bent verantwoordelijk voor het ondersteuningsproces rondom een aantal bewoners. Je stelt ondersteuningsplannen op, bewaakt doelen en bespreekt de voortgang met bewoners, verwanten, collega's en betrokken disciplines. Daarbij help je collega's om vanuit dezelfde afspraken en begeleidingslijn te werken. De verschillen tussen bewoners maken je werk afwisselend. Bij de ene bewoner stimuleer je zelfstandigheid met vooral verbale ondersteuning. Een ander help je intensiever bij dagelijkse vaardigheden of ADL. Steeds kijk je naar wat iemand zelf kan en waar ondersteuning nodig is. Je: - coördineert de ondersteuning rondom een aantal bewoners; - stelt ondersteuningsplannen op, bewaakt doelen en evalueert deze; - signaleert veranderingen in gedrag, gezondheid en welzijn; - stemt af met familie, het sociale netwerk, gedragsdeskundige en andere disciplines; - ondersteunt bij dagelijkse begeleiding, ADL en medicatie; - rapporteert zorgvuldig in het ECD; - stimuleert eigen regie, zelfstandigheid en het behouden van vaardigheden.
