Wat ga je doen In de gezellige wijk Bangert & Oosterpolder in Zwaag vind je De Hollandse Cirkel: een kleinschalige, betrokken woonlocatie met 20 appartementen waar cliënten zelfstandig wonen en een warme ontmoetingsruimte waar cliënten en begeleiding samenkomen. Alles wat je nodig hebt, ligt binnen handbereik: winkels, een zorgcentrum en plekken voor ontmoeting en een praatje. Je komt terecht in een betrokken en energiek team, waar je werkt als persoonlijk begeleider. We ondersteunen cliënten met een licht verstandelijke beperking, ieder met hun eigen verhaal, wensen en dromen. Bij ons draait het om één ding: een zo normaal mogelijk leven voor onze cliënten. Of het nu gaat om werk, relaties, een kinderwens of het opbouwen van een sociaal netwerk, jij helpt cliënten hun eigen pad te vinden. En gaat het even minder? Dan ben jij er. Als persoonlijk begeleider ben jij de spil rondom een aantal cliënten. Jij zorgt dat alles klopt en blijft aansluiten bij hun leven. Je zoekt ze actief op en weet met je flexibele houding en out of the box denken de juiste zorg te verlenen. Je stelt samen met de cliënt een persoonlijk zorgplan op; Je vertaalt wensen naar haalbare doelen; Je evalueert en stelt bij waar nodig; Je werkt nauw samen met collega’s, een orthopedagoog en andere disciplines; Je betrekt het netwerk en organiseert overleggen. Wat bieden wij Je komt te werken in regio Hoorn; via Amsterdam, Haarlem en Alkmaar minder dan 45 minuten rijden; Salaris in FWG 40 CAO GHZ; Opleidings- en ontwikkelingsmogelijkheden; Reiskostenvergoeding, 8% vakantiegeld, maar ook een eindejaarsuitkering van 8,33%; Faciliteiten om ‘goed in je vel’ te zitten; Extra arbeidsvoorwaarden, waaronder een fietsplan, korting op fitness en een computer/laptop/iPad regeling en het gebruik Benefits at Work; Leuke bonusregeling v

€ 350,- netto bij het aandragen van je nieuwe collega’s. Wat neem je mee Je beschikt over een diploma Zorg en Welzijn (niveau 4); Je staat stevig in je schoenen en kunt goed omgaan met een dynamische werkomgeving en ethische dilemma's; Je kunt de begeleiding goed afstemmen op de behoefte van de cliënt; Werkdagen van maandag t/m zondag; Je draait dag- avond- & slaapdiensten.
