Dan zoeken we jou. Jouw locatie Bij Krabbescheer wonen 24 bewoners met een lichte verstandelijke beperking (LVB) , vaak in combinatie met psychiatrische problematiek. Iedere bewoner heeft een eigen tweekamerappartement met keuken en badkamer. Veel bewoners gaan overdag naar werk of dagbesteding. Muziek speelt een belangrijke rol op de locatie: bewoners maken samen muziek in een eigen band. Ook zijn er activiteiten vanuit Vergroot je Wereld en is er contact met familie en de buurt. Jouw team Als Persoonlijk Begeleider LVB werk je in een jong en energiek team dat samen verder bouwt aan de locatie. Collega's denken met elkaar mee, staan voor elkaar klaar en houden de sfeer open, informeel en nuchter. Er is ruimte om initiatief te nemen, ideeën in te brengen en elkaar te helpen groeien in het vak. Jouw uitdaging Als Persoonlijk Begeleider LVB combineer je dagelijkse begeleiding met een coördinerende rol. Je stelt samen met bewoners hun ondersteuningsplan op, werkt aan persoonlijke doelen en houdt de voortgang in de gaten. Je helpt bewoners om zoveel mogelijk zelf te doen en keuzes te maken. Daarbij bied je structuur wanneer gedrag of psychiatrische problematiek het dagelijks leven ingewikkeld maakt. Je kent de bewoners goed en merkt wanneer gedrag, stemming of omstandigheden veranderen. Je stemt af met collega's, familie, behandelaren en andere betrokkenen en zorgt dat afspraken terugkomen in de dagelijkse begeleiding.
