Locatie Op locatie Kersentuin (Tuinhuis) in Woudenberg wonen 15 bewoners met een licht of matig verstandelijke beperking (LVB/MVB), lichte gedragsproblematiek en een lichte verzorgingsvraag. Iedere bewoner heeft een eigen appartement. Bewoners gaan naar werk of dagbesteding en maken gebruik van activiteiten in de omgeving. De locatie ligt dichtbij het centrum en een supermarkt en is goed bereikbaar. Jouw team Als Coördinerend Begeleider werk je in een divers team van 9 medewerkers. Samen zorgen jullie voor passende begeleiding bij een uiteenlopende zorgvraag. Je werkt samen met collega's en betrokkenen rondom de bewoners en hebt een coachende rol binnen het team. Het Tuinhuis is een Op weg met de Ander-locatie met een christelijke identiteit. Jouw uitdaging Als coördinerend begeleider combineer je de dagelijkse begeleiding van bewoners met een coördinerende rol. Je biedt structuur, ondersteunt bij lichte ADL en medicatie en begeleidt bewoners bij hun dagelijkse leven. Je rapporteert in het ECD, stelt ondersteuningsplannen en doelen op en coördineert het zorgproces. Door de verschillende ondersteuningsvragen kan de mate van zelfstandigheid per bewoner verschillen. Op de ene verdieping wonen bewoners die relatief zelfstandig zijn, terwijl op de andere verdieping meer nabijheid en sturing nodig is. Je bewaakt de voortgang en kwaliteit van de ondersteuning, stemt af met betrokkenen en helpt collega's om vanuit dezelfde afspraken en doelen te werken.
