Heb je ervaring met bijkomende problematieken zoals verslaving of gedragsproblemen? Kom dan werken als Persoonlijk Begeleider lvb+ op onze woonlocatie Lage Nieuwstraat, midden in het leven van Den Haag! Dit ga je doen Als persoonlijk begeleider (begeleider 4) ben jij de stabiele factor voor onze bewoners. Jij helpt hen bij het vinden van rust, regelmaat en structuur in hun leven, zodat ze zich kunnen richten op hun persoonlijke doelen en ontwikkeling. Je werkt intensief samen met onze bewoners, leert hen goed kennen en biedt hun de juiste begeleiding op het gebied van zelfredzaamheid, sociaal gedrag en het dagelijks leven. Maar jouw werk gaat verder dan dat: - Je werkt nauw samen met familieleden, vrienden en professionele partners, zoals ggz, politie, reclassering en verslavingszorg. - Je draagt verantwoordelijkheid voor het opstellen en uitvoeren van zorg- en begeleidingsplannen, aangepast aan de specifieke behoeften van de bewoners. - Je werkt met het Middin Kompas en volgens de LifeWise-methodiek, waarbij je een leerprogramma volgt om je vaardigheden verder te ontwikkelen. Hier ga je werken Op onze woonlocatie de Lage Nieuwstraat werk je midden in het leven van de binnenstad in Den Haag. Je gaat aan de slag op een woonlocatie waar zo'n 16 mensen met een licht verstandelijke beperking wonen, waarvan 8 in crisisopvang. De bewoners hebben te maken met diverse problematieken zoals verslavingen, autisme of psychiatrische stoornissen. Elke situatie vraagt om een andere aanpak, zonder standaardoplossingen, maar telkens weer een nieuwe uitdaging waarbij jouw expertise gevraagd wordt. Je werkt hier: - Binnen een divers team, waar je je zeven collega's met wie je samenwerkt ook steunt en stimuleert. Ook met een jobcoach en gedragsdeskundige werk je nauw samen. - Met een diverse en soms complexe doelgroep...
