Bewoners wonen straks zelfstandig in ruime appartementen met eigen sanitair en balkon. Het gebouw staat er bijna; het dagelijks leven moet hier nog vorm krijgen. Bewoners brengen ieder hun eigen verhaal, gewoontes en ondersteuningsvraag mee, soms gecombineerd met somatische, GGZ- of verslavingsproblematiek. Jouw team Als persoonlijk begeleider LVB Ouderen werk je in een nieuw team van begeleiders, Verzorgenden IG en verpleegkundigen. Een deel kent Philadelphia al, anderen starten hier nieuw. Samen met de manager en gedragsdeskundige bouw je aan een duidelijke manier van werken. Je verbindt verschillende disciplines, bewaakt de inhoudelijke lijn en helpt collega's om afspraken te vertalen naar de dagelijkse praktijk. Jouw uitdaging Als persoonlijk begeleider LVB Ouderen combineer je dagelijkse begeleiding met een stevige coördinerende rol. Je bent de verbindende schakel tussen bewoners, collega's, behandelaren en hun netwerk. Je coördineert de ondersteuning van bewoners, houdt overzicht over ondersteuningsplannen en zorgt dat afspraken worden uitgevoerd. Ook bereid je MDO's voor, zit je deze waar nodig voor en vertaal je besluiten naar concrete acties. Omdat de locatie nieuw is, bouw je ook mee aan de basis.
