Persoonlijk Begeleider LVB/VB | Delft Bij Philadelphia heet deze functie officieel Coördinerend Begeleider. Wil jij dichtbij bewoners staan én verantwoordelijkheid nemen voor de kwaliteit van hun begeleiding? Op Hoornse Zoom in Delft combineer je dagelijkse begeleiding met coördinatie en werk je met volwassenen en ouder wordende mensen met een lichte tot matige verstandelijke beperking. Voor Hoornse Zoom in Delft zoeken we een Persoonlijk Begeleider voor 24 tot 36 uur per week . Jouw rol Als Persoonlijk Begeleider sta je midden in het dagelijks leven van de bewoners én houd je overzicht over de begeleiding rondom een aantal cliënten. Je kent de bewoners goed. Daardoor merk je het wanneer gedrag verandert, iemand minder lekker in zijn vel zit of juist toe is aan een volgende stap. Samen met de cliënt, verwanten, collega's en andere betrokken professionals vertaal je die signalen naar passende ondersteuning. Je bent verantwoordelijk voor het opstellen en evalueren van ondersteuningsplannen en zorgt ervoor dat afspraken niet alleen in het dossier staan, maar ook echt terugkomen in de dagelijkse begeleiding. Je: - coördineert de begeleiding rondom een aantal cliënten en bewaakt hun persoonlijke doelen; - stelt ondersteuningsplannen op, evalueert deze en vertaalt afspraken naar de dagelijkse praktijk; - stimuleert bewoners om zoveel mogelijk zelf keuzes te maken en zelfstandig te blijven; - signaleert veranderingen in gedrag, welzijn of ondersteuningsbehoefte en onderneemt hierop actie; - onderhoudt contact met verwanten, behandelaren en andere betrokken professionals; - ondersteunt bewoners bij dagelijkse routines, sociale situaties en waar nodig lichte ADL; - helpt collega's om samen één duidelijke begeleidingslijn te houden en rapporteert zorgvuldig in het ECD. Je staat dus niet vanaf de zijlijn te coördineren. Juist doordat je zelf meewerkt op de locatie en bewoners goed kent, kun je de regie rondom hun begeleiding goed pakken...
