Grensarbeid zoekt voor de Stichting Radar een persoonlijk begeleider Meezenbroek.

Dit is hoe jouw werkdag uit ziet

De wekker gaat, je vertrekt naar werk. Jouw werkdag start vroeg met het wekken en verzorgen van onze cliënten. In totaal wonen 20 ouder wordende cliënten met een beperking op onze woonvorm. Na het wekken, volgt het ontbijt. Een moment van samen zijn. Cliënten die naar de dagbesteding extern gaan, help je op weg.

Om 9:00 uur verzamelen jij en je collega's zich op kantoor voor de overdracht. Jullie bespreken de bijzonderheden van de afgelopen dagen en vandaag. Op Meezenbroek is een dagbesteding op locatie. De dagbesteding medewerker start na de overdracht met opstarten van de dagbesteding en jij bent bezig met telefoontjes, post, afspraken begeleiden of gewoon een goed gesprek met een van de cliënten. Je houdt je ook bezig met de informele zorg. Jij bent de schakel tussen onze locatie en de wijk en regelt wat de wijk kan betekenen voor onze cliënten.

Rond 12:00 uur is het lunchtijd. Samen met de cliënten lunch je aan tafel. Er wordt gekletst en soms gewoon in stilte genoten. Na de lunch gaan de cliënten verder met de activiteit op de dagbesteding. Jij gaat verder met de telefoontjes, post, het begeleiden van afspraken en de één-op-één contactmomenten met cliënten. Rond 16:00 uur komen de cliënten die extern naar de dagbesteding zijn geweest terug, vaak met verhalen en blije gezichten.

Je werkt in een team van 23 collega's, waaronder ook vrijwilligers en stagiaires. Gedurende de dag kom je je collega's regelmatig tegen en door het portofoonsysteem sta je altijd met elkaar in contact.

Aan het einde van de middag neemt het avonddienst team de dienst over. Zij koken en eten samen met de cliënten en collega's. Er is tijd om met elkaar in gesprek te gaan en de dag te bespreken. Daarna volgt de avondzorg. Sommige cliënten hebben alleen aansturing tot verzorging nodig, anderen hulp bij het wassen. De dag wordt afgesloten met een gezamenlijk koffiemoment met de cliënten in een huiselijke sfeer.

Voor het einde van elke dienst wordt afgestemd met collega's en worden de werkzaamheden gerapporteerd.

Omdat onze cliënten 24/7 begeleiding nodig hebben, werk je in wisselende diensten, inclusief weekenden en slaapdiensten. We vinden het belangrijk om rekening te houden met jouw privé situatie. In samenspraak met het team stemmen we af hoe we de beste begeleiding kunnen bieden en rekening kunnen houden met jou als individu.

  • Een diploma van een opleiding in sector zorg en welzijn op minimaal mbo-niveau 3. Denk aan SPW, MMZ, verzorgende IG, verpleegkundige, social work enz
  • Affiniteit met de ouder wordende cliënt

  • Actief in het betrekken van de buurt en wijk bij de zorg voor onze cliënten
  • Positief, teamspeler
  • Snel schakelen in verschillende ondersteuningsvragen
  • Sociale vaardigheden: tact, inlevingsvermogen en motivatiekracht
  • Regie voeren bij opstellen, uitvoeren, bijstellen en reflecteren op ondersteuningsplannen (vanaf mbo-niveau 4)
  • Lef, eigenaarschap en nieuwsgierigheid
  • Bereid tot het werken in een onregelmatig dienstrooster
