The posting

Maak echt verschil in het leven van mensen met MS – samen met een hecht team dat voor elkaar en de cliënt gaat. Het Oosterveld MS is een zorg intensieve nieuwe woonlocatie voor mensen met Multiple Sclerose. Hieraan gekoppeld is het Regionaal Expertise Centrum MS (REC) van Siza gespecialiseerd in de zorg en behandeling van mensen met MS en ernstige functionele beperkingen en aanzienlijke gezondheidscomplicaties. De zorg is gericht op het aanbrengen en onderhouden van een dag structuur. We ondersteunen ook bij de ADL (algemene dagelijkse levensverrichtingen) en de (VRV) verpleegtechnische zorg. Daarnaast ondernemen we gezamenlijke activiteiten met de bewoners. Bijvoorbeeld bij activering en met sporten bij FitLife op Het Dorp. Het verwacht aantal uur voor deze functie ligt tussen de 8 - 36 uur per week. Jouw team Je komt terecht in een warm en betrokken team waarin we elkaar helpen, ondersteunen en ook plezier maken. Samen dragen we de verantwoordelijkheid voor de beste zorg aan onze cliënten, en we vinden het belangrijk om oog te hebben voor elkaar.Zo maak jij het verschil Als Persoonlijk Begeleider ben je een belangrijke spil in het dagelijks leven van onze cliënten. Je begeleidt, ondersteunt en stimuleert hen bij wat zij wél kunnen en helpt hen omgaan met de gevolgen van MS. Naast zorg en ondersteuning vinden we het belangrijk tijd en aandacht te besteden aan het opbouwen van een band met onze cliënten, zodat we hen zo prettig mogelijk kunnen ondersteunen in ieder zijn of haar proces. Wat je doet: • Je biedt persoonlijke en professionele zorg op maat. Hierbij kun je denken aan het bieden van een veilige leefomgeving, ondersteuning bij de ADL en met verpleegtechnisch handelen, signaleren van veranderingen in de gezondheid en hierop adequaat inspelen • Je werkt intensief samen in een multidisciplinair team én de client en zijn/haar netwerk • Je helpt cliënten bij het behouden van hun zelfstandigheid én kwaliteit van leven • Je brengt jouw kritische blik én
