Persoonlijk begeleider Ouderen voor 28-32 uur - Beschermd Wonen - Appartementen - 55-85 jaar Een vleugel met 29 bewoners in een appartementencomplex in Maassluis. Daar ga je aan de slag als jij kiest voor het werken met mensen van 55-85 jaar die bij ons beschermd wonen. Zij hebben ieder een eigen appartement en psychische of psychiatrische klachten. Jij komt bij hen thuis en je werkt met hen in de gezellige, gezamenlijke woonkamer. In deze huiselijke omgeving heb jij met hen een professionele band. Jij maakt het mogelijk De oudere bewoners ervaren structuur en blijven of worden zo zelfstandig mogelijk. Zij ervaren dat hun leven fijner wordt en meer betekenis krijgt. Jij zorgt hiervoor door: - methodisch te werken en de verantwoordelijkheid te nemen voor de dossieropbouw van 4-7 bewoners. - samen met de bewoners een individueel begeleidingsplan, inclusief doelen, op te stellen en uit te voeren. Hiermee werken jullie toe naar zingeving, meedoen in de gemeenschap en uitbreiding van hun competenties en sociale contacten. - tijdens jouw dienst aanspreekpunt te zijn voor alle bewoners, eventuele bezoekers en mensen die ons via mail of telefonisch benaderen. Het ervaringsverhaal van bewoner René geeft je een nog completer beeld van wat je voor onze bewoners kunt betekenen. Hier maak jij het waar De organisatie-eenheid Wonen & Begeleiding bestaat uit 2 teams. In totaal werken hier 16 betrokken professionals als nachtcoach, (persoonlijk) begeleider, verpleegkundige en manager. Manager Claudia is recent bij het team gestart. Samen met de teams zorgt ze met vernieuwde energie voor de ontwikkeling van zowel de bewoners, de begeleiding als het team. De 2 teams zijn verdeeld over 3 locaties actief. Zo is er een groepswoning voor volwassen, appartementen in een vleugel van een seniorencomplex en appartementen op 'losse' locaties. In totaal zijn er 44 bewoners.
