Persoonlijk Begeleider Ouderenzorg VB - Gehandicaptenzorg Gorinchem Voor locatie Phila Rank in Gorinchem zoeken wij een Coördinerend Begeleider voor 24 uur per week, meer uren zijn bespreekbaar. Werk jij graag met ouder wordende cliënten met een verstandelijke beperking? Zoek je een functie waarin je zowel coördineert als meewerkt op de groep? Dan voel jij je waarschijnlijk snel thuis op Phila Rank. Jouw rol Als Coördinerend Begeleider/ Persoonlijk Begeleider ben jij de stabiele factor binnen de woning. Je bewaart het overzicht, houdt de zorgkwaliteit scherp in de gaten en bent tegelijk zichtbaar aanwezig voor cliënten en collega's. Je werkt met ouder wordende cliënten (50+) met een verstandelijke beperking. Sommige bewoners worden lichamelijk kwetsbaarder, anderen hebben meer behoefte aan structuur, nabijheid of duidelijkheid. Jij ziet wat iemand nodig heeft en weet daarin rust en vertrouwen te brengen. Je bent verantwoordelijk voor het zorgproces rondom cliënten en zorgt dat alles goed op elkaar aansluit. Van ondersteuningsplannen tot afstemming met familie, mentoren en behandelaren: jij houdt de lijnen kort en helder. Daarnaast werk je gewoon mee op de groep. Juist die combinatie maakt deze functie sterk. Je staat niet op afstand te coördineren, maar weet precies wat er speelt. Dit ga je doen: - Coördineren van het zorgproces rondom cliënten - Opstellen, bewaken en evalueren van ondersteuningsplannen - Ondersteunen van cliënten bij dagelijkse keuzes en zelfstandigheid - Afstemmen met familie, mentoren en behandelaren - Bewaken van kwaliteit van zorg en begeleiding - Collega's ondersteunen in structuur en samenwerking - Meewerken op de woning en overzicht behouden - Organiseren, prioriteren en helder communiceren Binnen de locatie werken jullie onder andere met Active Support en de visie "Vergroot je wereld".
