The posting

Op Struiklaan 1-5 wonen tien bewoners met een ernstige meervoudige beperking (EMB). De bewoners verschillen in leeftijd, ondersteuningsvraag en ontwikkelingsfase. Waar de één nog volop in ontwikkeling is, geniet de ander juist van rust en structuur in een latere levensfase. Overdag gaan alle bewoners naar hun eigen dagbesteding en binnen de woning zijn er twee gezellige huiskamers waar ontmoeting en begeleiding centraal staan.

De begeleiding bestaat uit een combinatie van zorg, ondersteuning en het creëren van een veilige en prettige leefomgeving. Juist die afwisseling tussen verzorging en begeleiding maakt het werk dynamisch en betekenisvol. Jij weet goed aan te sluiten bij wat een bewoner nodig heeft en kunt schakelen tussen verschillende ondersteuningsvragen.

Soms zitten bewoners minder goed in hun vel. Op die momenten bied jij rust, duidelijkheid en nabijheid. Met humor, geduld en aandacht weet jij bewoners weer op weg te helpen. Je bent onvoorwaardelijk aanwezig en houdt continu verbinding met de bewoners.

Als persoonlijk begeleider heb je daarnaast een coördinerende rol. Je stelt zorgplannen op, bewaakt de voortgang en zorgt voor continuïteit in de ondersteuning. Je werkt hierbij nauw samen met collegas, verwanten en andere disciplines. In je dagelijkse werk word je ondersteund door een zorgcoördinator, gedragskundige en zorgmanager.

Je komt terecht in een team dat volop in ontwikkeling is en continu werkt aan het verhogen van de kwaliteit van zorg. Jouw frisse blik, initiatief en professionele houding worden daarin gewaardeerd.
