In Rotterdam werken meerdere teams en daarvoor zijn we opzoek naar planners voor 24, 32 en 40 uur per week. Als planner draait je werkdag om het maken van de best mogelijke match in de planning tussen onze cliënten en thuishulpen. Dagelijks heb je telefonisch contact en schakel je met beide groepen. Zo breng jij de wensen in kaart en zorg je voor een optimale planning voor 60 tot 120 thuishulpen. (afhankelijk van het aantal uren dat je per week werkt) Werken bij Tzorg is zoveel meer. Zorg die mensen helpt om zelfstandig te blijven wonen, in hun eigen vertrouwde omgeving. Of het nu gaat om ondersteuning bij het huishouden of persoonlijke begeleiding - wij zijn er met aandacht, betrokkenheid en een warm hart. Samen handelen we uit voorzorg en zorgen we voor een aangenaam thuis. Werkzaamheden planner: - Jij maakt een optimale planning voor 60 tot 120 thuishulpen, waarbij je rekening houdt met contracturen, routes en de vraag van de organisatie of cliënt. Deze planning maak je in ons digitale plansysteem ONS. - Je werkt 4 dagen op kantoor. Zo behoud je het contact en een goede samenwerking met je collega's. - Je houdt dagelijks telefonisch contact over de planning met cliënten en thuishulpen. Je weet deze gesprekken goed te sturen, blijft rustig onder druk en communiceert helder en doelgericht. - Vooruitwerken is als planner erg belangrijk. Je anticipeert op veranderingen en plant proactief om de continuïteit van de zorg te waarborgen. - Je coördineert schriftelijke communicatie vanuit kantoor naar thuishulpen en cliënten. Denk bijvoorbeeld aan e-mails en brieven over veranderingen in de planning. - Je werkt nauw samen met je zorgcoördinator en hebben...
