Wegens de groei van onze organisatie zoeken wij een ervaren Planner Revalidatiegeneeskunde , die onze multidisciplinaire teams wil ondersteunen. De afdeling planning is het organisatorische hart van onze revalidatiecentra. Hier worden alle behandeltrajecten gecoördineerd en zorgvuldig op elkaar afgestemd, zodat cliënten op het juiste moment de juiste zorg ontvangen. Wat ga je doen? Bij TopzorgGroep werk je aan een gezonder en vitaler Nederland. Jij staat klaar voor mensen die vooruit willen en samen zorgen we voor topzorg. Als planner ben je verantwoordelijk voor het efficiënt en zorgvuldig coördineren en afstemmen van multidisciplinaire revalidatietrajecten waarbij je de cliëntbehoefte, behandelcapaciteit en organisatorische randvoorwaarden samenbrengt in een sluitende planning Dit worden jouw taken: - Het plannen van revalidatietrajecten die voor 3 maanden vooruit worden gepland waarbij verschillende disciplines worden ingezet; - Cliënten te woord staan bij vragen en zorgt ervoor dat deze zo goed mogelijk worden beantwoord; - Het plannen van afspraken bij de behandelaren voor (met name) de tweedelijnszorg; - Het bedienen van meerdere locaties en regio's en dit overzicht goed bewaren; - Het plannen van afspraken op een zo efficiënt mogelijke manier in de agenda's bij de behandelaren.
