Huisartsenpraktijk Woldendorp zoekt een POH Voor Huisartsenpraktijk Woldendorp zoeken wij vanaf 1 januari 2027 een enthousiaste Praktijkondersteuner (POH) voor 3 dagen per week . Een kleine dorpspraktijk, korte lijnen, fijne collega's en patiënten die je écht leert kennen. Klinkt dat als een werkplek voor jou? Werken in Woldendorp Wij zijn een huisartsenpraktijk op het Groningse platteland. Een praktijk waar we onze patiënten kennen en waar persoonlijke aandacht nog echt belangrijk is. Ook binnen ons team houden we van korte lijnen. We werken prettig samen, helpen elkaar wanneer dat nodig is en vinden een fijne werksfeer belangrijk. Geen grote organisatie met veel lagen, maar een praktijk waarin je elkaar makkelijk vindt en waarin jouw ideeën en inbreng welkom zijn. Wat ga je doen? Als POH heb je je eigen spreekuren en begeleid je patiënten met onder andere: - Diabetes mellitus - COPD en astma - Cardiovasculair risicomanagement (CVRM) - Atriumfibrilleren (AF) - Leefstijl en preventie Je werkt zelfstandig, maar zeker niet alleen. Overleggen met de huisarts of even sparren met een collega kan altijd. Wie zoeken we? We zoeken vooral een fijne collega die bij onze praktijk past. Iemand die betrokken is bij patiënten, zelfstandig kan werken en het leuk vindt om onderdeel te zijn van een klein team. Je bent praktisch ingesteld, denkt graag mee en houdt van persoonlijk contact. Je hebt een afgeronde opleiding tot POH. Ervaring is natuurlijk mooi meegenomen, maar voor ons is een goede klik minstens zo belangrijk. Wat krijg je van ons?
