Wil jij als POH-GGZ een belangrijke rol spelen in de eerstelijns GGZ en patiënten helpen om weer grip te krijgen op hun psychische gezondheid? Dan maken we graag kennis met je! De functie Als POH-GGZ ben je een vertrouwd en laagdrempelig aanspreekpunt binnen de huisartsenpraktijk voor patiënten met psychische en psychosociale klachten. Je voert zelfstandig gesprekken en spreekuren en onderzoekt samen met de patiënt wat er precies speelt en wat er nodig is. Je brengt de klachten, situatie en hulpvraag zorgvuldig in kaart, maakt een inschatting van de benodigde ondersteuning en biedt passende, kortdurende begeleiding. Daarbij kijk je niet alleen naar de klachten, maar juist ook naar de mogelijkheden, veerkracht en eigen regie van de patiënt. Je krijgt te maken met een veelzijdige doelgroep en uiteenlopende hulpvragen, zoals: - angst- en stemmingsklachten; - stress en overbelasting; - rouw en verlies; - problemen in het dagelijks functioneren; - psychosociale problematiek; - langdurige of chronische psychiatrische problematiek. Je helpt patiënten om meer inzicht en overzicht te krijgen, geeft psycho-educatie en werkt samen aan concrete en haalbare stappen. Ook besteed je aandacht aan terugvalpreventie en het behouden van stabiliteit. Bij patiënten met langdurige of chronische psychiatrische problematiek houd je waar nodig contact, bijvoorbeeld door middel van een huisbezoek. Je werkt hierbij nauw samen met de huisarts en andere betrokken zorgverleners. Wanneer aanvullende behandeling of ondersteuning nodig is, denk je actief mee over de juiste vervolgstap. Je kent de sociale kaart en onderhoudt contact met relevante organisaties en zorgverleners in de regio, zodat patiënten zo goed mogelijk naar passende hulp kunnen worden doorverwezen. Goede ...
