Logo GGZ Friesland 52 POH GGZ HBO, WO Minimaal 8 uur Locatie Friesland Sluitingsdatum: 1 november 2026 Geplaatst op: 7 oktober 2026 Als praktijkondersteuner geestelijke gezondheidszorg, oftewel POH GGZ, ondersteun je huisartsenpraktijken bij het verlenen van zorg aan patiënten met psychische en psychosociale klachten. Binnen enkele gesprekken signaleer, diagnosticeer en intervenieer je bij de problemen en hulpvragen van de patiënt. Je biedt behandeling of begeleiding op een snelle, vakkundige en laagdrempelige manier en indien nodig verzorg je een doorverwijzing naar ketenpartners waar nauw mee wordt samengewerkt. Door deze versterking van de eerstelijns zorg is de zorg makkelijk toegankelijk, wordt zelfmanagement door patiënten gestimuleerd en vinden er minder verwijzingen plaats naar de basis en specialistische GGZ. Je komt in dienst van GGZ Friesland, maar gaat aan de slag bij één of meerdere huisartsenpraktijken door middel van een detachering. Hierbij verbind je je meestal duurzaam en voor langere tijd aan dezelfde praktijk. Je werkt veelal zelfstandig, maar hebt daarnaast veel contact met je collega's van de huisartsenpraktijk en je collega's van GGZ Friesland. Dit laatste krijgt vorm door gezamenlijke intervisiebijeenkomsten, opleidingen en werkoverleggen. Op deze manier maak jij onderdeel uit van een groter geheel en heb je de mogelijkheid terug te vallen op collega's en van collega's te leren.
