POH GGZ Oisterwijk Huisartsen Oisterwijk kliniek Moergestelseweg 32H Oisterwijk Per 01 January 2027 16 uur per week Huisartsen Oisterwijk kliniek zoekt; per 1 januari 2027; POH-GGZ , 16 uur in de week Door pensionering van één van onze POH's-GGZ zijn wij per ingang van 1 januari 2027 op zoek naar een nieuwe collega voor 16 uur in de week. Wij zijn een gezellige HOED met vier solo huisartsenpraktijken onder 1 modern dak in Oisterwijk. Je zal werkzaam zijn in een divers team van huisartsen, assistentes en andere praktijkondersteuners (somatiek, ouderenzorg en GGZ). Functieomschrijving: - Als POH-GGZ bied je zorg aan patiënten van alle leeftijden met psychische en psychosociale problemen. Tot je taken horen: - Zelfstandig verlenen van laagdrempelige geestelijke gezondheidszorg binnen de huisartsenpraktijk. - Signalering, probleemverheldering, diagnostiek, psycho-educatie, kortdurende behandeling/begeleiding, eventueel doorverwijzing en nazorg, ondersteund door middel van eHealth. - Samenwerken je collega POH GGZ en met huisartsen door middel van afstemming en overleg. - Je zorgt dat je op de hoogte bent van de sociale kaart en zoekt de samenwerking met andere zorgverleners in het sociale domein. - Verzorgen van consultaties en bijdragen aan preventieve activiteiten en voorlichting. Functie-eisen: - Je voldoet aan het functie-en-competentieprofiel voor de POH-GGZ van de Landelijke Huisartsen Vereniging. - Registratie in het Kwaliteitsregister POH-GGZ. - Afgestudeerd als psycholoog of psychiatrisch verpleegkundige met minimaal 2 jaar ervaring - Goede communicatieve vaardigheden; voor een goede afstemming met patiënten en netwerkpartners. - Zelfstandigheid en flexibiliteit; een verantwoordelijke functie met vrijheid om maatwerk te leveren - Kennis van diverse behandelmethoden en psychische stoornissen. - Het is fijn als je ervaring hebt met e-health en het werken in de huisartsenzorg Wat bieden wij: - De arbeidsvoorwaar...
