Ga snel naar: Quick facts - Functie: Praktijk Ondersteunend Hulpverlener - Somatiek (POH-S) - Locatie: Amsterdam - Salaris: 18,24 euro per uur - Start: in overleg Jouw rol Als POH-S begeleid je patiënten met chronische aandoeningen zoals diabetes, astma/COPD en hart- en vaatziekten. Je houdt je eigen spreekuur, voorziet patiënten van heldere voorlichting over hun ziekte, begeleidt bij medicatiegebruik, stelt dit zo nodig bij en stimuleert positieve veranderingen in de leefstijl. Ook voer je controle onderzoeken uit om de gezondheid van patiënten te monitoren. Deze gevarieerde taken dragen bij aan een persoonlijke en effectieve zorgverlening binnen deze boeiende rol. Verder ontlast je de huidige huisartsenzorg en zorg je ervoor dat de reguliere huisartsenzorg goed draaiende kan worden gehouden. Daarnaast is de vacature als POH-S in Groningen een ontzettend leerzame ervaring! Wat bieden we?
