POH-S bij Huisartsenpraktijk Star - Capelle aan den IJssel - 8-24 uur Voor mijn praktijk zoek ik een GEDIPLOMEERD POH-SOMATIEK (m/v - 8 tot 24 uur per week) De huisartspraktijk bevindt zich in een klein gezondheidscentrum en bestaat uit een praktijk van 2500 patiënten op het verstedelijkt platteland in Capelle aan den IJssel, waar het persoonlijk contact met patiënten hoog in het vaandel staat. Met ons motto: ' we doen het samen!'' . De huisartsenpraktijk is aangesloten bij Rijnmond Dokters. We werken met Medicom als HIS. Korte samenvatting van de functie Wij zijn op zoek naar een ervaren en gediplomeerde praktijkondersteuner somatiek voor 1 of 2 of 3 dagen per week. Dagen en tijden zijn in overleg. U draait zelfstandig spreekuren, die gericht zijn op de chronische patiënten populaties. Tot het takenpakket behoren o.a.: - Diabetesspreekuur; - Astma/COPD spreekuur met longfunctiemetingen; - CVRM spreekuur; - ECG's maken; - Advies SMR (stoppen met roken); - Doppler onderzoek/EAI; - 30 minuten bloeddrukmeting; - Ouderenzorg. Wat zoeken we in jou? Wij zijn op zoek naar een zelfstandig werkende POH-S, die het eigen werk goed kan organiseren. Daarnaast kunt u goed samenwerken met een multidisciplinaire team.
