Locatie: Lent (Nijmegen-Noord) Startdatum: 1 januari 2027 In verband met het vertrek van een van onze praktijkondersteuners zijn we op zoek naar een betrokken en enthousiaste praktijkondersteuner somatiek voor 24 uur per week. Ben jij een POH-S die graag zelfstandig werkt, oog heeft voor de mens achter de aandoening en het leuk vindt om samen met collega's de zorg steeds een beetje beter te maken? Dan maken we graag kennis met jou! Over Thermion Wij zijn een moderne groepspraktijk in een licht en ruim pand, op fietsafstand van het centrum van Nijmegen. Met 8 huisartsen, 9 doktersassistentes, 5 praktijkondersteuners (2 somatiek en 3 GGZ), een praktijkmanager, financieel medewerker en 2 huisartsen in opleiding vormen we een hecht, energiek team met korte lijnen en een warme sfeer. Waar we voor staan We bieden persoonlijke, laagdrempelige zorg van hoge kwaliteit aan 11.500 patiënten. Dat doen we in samenspraak met onze patiënten en in nauwe samenwerking met collega zorgverleners in de nulde, eerste en tweede lijn. Onze patiëntenzorg combineren we met wetenschappelijk onderzoek en onderwijs/opleiding. Benieuwd naar onze mensen en onze praktijk? Neem alvast een kijkje op www.huisartsenthermion.nl Jouw rol Als POH-S ben je samen met je collega verantwoordelijk voor de zorg aan patiënten met chronische aandoeningen en ouderen. Je voert zelfstandig spreekuren, begeleidt patiënten en denkt mee over passende zorg. We vinden het belangrijk dat je luistert, meedenkt en samen met de patiënt kijkt wat werkt. En als je een goed idee hebt om iets slimmer of beter te organiseren, horen we dat ook graag. Wat je o.a. gaat doen: • begeleiden en controleren van patiënten met chronische aandoeningen; • in kaart brengen en begeleiden van (kwetsbare) ouderen. • uitvoeren van controles en periodieke evaluaties; • geven van voorlichting en leefstijladviezen; • signaleren van problemen en zo nodig afstemmen met de huisarts of andere zorgverleners; ...
