The posting

Wegens groei van de praktijk en toekomstige urenafbouw van een van onze praktijkondersteuners zijn wij op zoek naar een enthousiaste POH-somatiek. Over ons Onze praktijk heeft 4600 patiënten en is gevestigd in een medisch centrum in Zierikzee met nog een andere huisartspraktijk en diverse eerstelijns disciplines. Wij bieden allround huisartsgeneeskundige zorg, die vertrouwd en persoonlijk is. Ons team bestaat uit 4 huisartsen, 3 doktersassistentes en 2 praktijkondersteuners-somatiek. Jouw werkzaamheden We zijn op zoek naar een enthousiaste POH die spreekuren doet in het kader van ketenzorg Diabetes, Astma/COPD (inclusief longfuncties), CVRM en ouderenzorg. Je begeleidt patiënten bij leefstijlveranderingen en werkt direct samen met huisartsen en andere zorgverleners binnen de praktijk. Daarnaast zullen we je vragen af en toe wat eenvoudige assistentetaken op je te nemen. Wie ben jij? • Je hebt een afgeronde POH-S opleiding en bij voorkeur enige werkervaring. • Je werkt graag zelfstandig en hebt goede communicatieve vaardigheden. • Je houdt goed overzicht en werkt gestructureerd. • Je bent betrokken, leergierig en gemotiveerd om je te ontwikkelen. • Je bent patiëntgericht, proactief en flexibel. • We nodigen je ook uit te solliciteren als je de opleiding tot POH-S wilt volgen en hiervoor een stageplek zoekt met uitzicht op een vaste functie nadien.
