- 24 uur per week - Hoofddorp Gezondheidscentra Haarlemmermeer (SGH) bestaat uit drie gezondheidscentra en de HuisArtsenspoedpost Haarlemmermeer. Met ruim 150 collega's bieden we eerstelijnszorg van hoge kwaliteit. In ons geheel vernieuwde Gezondheidscentrum Overbos is een vacature ontstaan voor een: POH somatiek en ouderenzorg 24 uur per week GC Overbos staat midden in een dynamische wijk met een gevarieerde populatie. De patiëntenzorg staat op de eerste plaats. Samen met nog twee collega's POH-S, zes huisartsen, ondersteuning van assistentes, POH-beweegzorg en POH-GGZ volwassenen en jeugd wordt eerstelijnsgezondheidszorg geboden aan ruim 8000 patiënten. Er is een nauwe samenwerking met de andere twee gezondheidscentra. Wat ga je doen? Je voert het vak POH-S in de volle breedte uit. Tot je taken behoren onder andere het zelfstandig zorginhoudelijk ondersteunen en begeleiden van patiënten met een chronische aandoening (bijvoorbeeld DM, CVRM en astma/COPD) en het verlenen van ouderenzorg. Je werkt in een multidisciplinair team van zorgprofessionals, waar de lijnen kort zijn.
