Je behandelt uiteenlopende fysieke en mentale zorgvragen en begeleidt patiënten met chronische aandoeningen. Hierbij geef je voorlichting en advies over zelfzorg en gezondheid, afgestemd op de diverse culturele achtergronden van de doelgroep. Vaak werk je met ondersteuning van een tolk om de zorgvraag helder te krijgen. Je werkt nauw samen met een multidisciplinair team, waaronder huisartsen, doktersassistenten en POH-GGZ, om de zorg optimaal te coördineren. Daarnaast stem je af met ketenpartners zoals het COA, apotheken en specialisten. Flexibiliteit is belangrijk, omdat je op verschillende locaties werkt. Samen met je collega's draag je bij aan een betrokken en professioneel team, waarin overleg en samenwerking centraal staan.
