Als praktijkadviseur bij HCWB ben je een verbindende schakel tussen praktijk, (zorg)programma's, projecten en innovaties. Je bent een vertrouwd aanspreekpunt voor praktijken en een herkenbaar gezicht in de regio. Functieomschrijving De functie Als praktijkadviseur ben je het visitekaartje van HCWB en ben je breed inzetbaar. Naast chronische zorg richt je je op ouderenzorg, mentale gezondheid en preventie & leefstijl. Geleidelijk ga je ook bijdragen aan de implementatie van andere HCWB-brede projecten en innovaties. Zo help je praktijken om samenhang te zien, keuzes te maken en veranderingen duurzaam in te bedden. Wat ga je doen? - Je bezoekt en adviseert huisartspraktijken over de uitvoering en doorontwikkeling van de zorgprogramma's, zoals chronische zorg, ouderenzorg, mentale gezondheid en preventie & leefstijl. - Je helpt praktijken om zorginhoudelijke afspraken, protocollen en werkwijzen goed te vertalen naar de dagelijkse praktijk. - Je bewaakt samen met praktijken de kwaliteit en samenhang binnen de zorgprogramma's en stimuleert verbeteringen waar nodig. - Je signaleert kansen en knelpunten en vertaalt deze naar gerichte verbeteracties en monitort de opvolging. - Je werkt intensief samen met kaderhuisartsen, programmamanagers, praktijkmanagers en andere collega's en neemt deel aan expertteams, kwaliteitscommissies en projectgroepen. - Je levert input voor scholingen, werkinstructies, communicatie en ondersteunende materialen die bijdragen aan de kwaliteit van goede en uniforme zorg in de regio.
