Ervaring is natuurlijk welkom, maar ook wanneer jenog in opleiding bent tot praktijkondersteuner nodigen we je graag uit om te reageren.Als POH-S begeleid je patiënten met chronische aandoeningen en lever je eenbelangrijke bijdrage aan de kwaliteit en continuïteit van de zorg binnen de praktijk. Jehoudt zelfstandig spreekuur en werkt daarbij nauw samen met de huisartsen en anderecollega's. Je richt je onder andere op de zorgprogramma's: DM;•CVRM;•Astma en COPD. • Maar deze praktijk zoekt meer dan alleen iemand die een goed spreekuur kan draaien.Er wordt voortdurend gekeken hoe de zorg slimmer en gemakkelijker georganiseerd kanworden. Digitalisering en innovatie spelen daarbij een belangrijke rol. Vind jij het leuk omdaarin mee te denken, nieuwe mogelijkheden uit te proberen en samen met het teamverbeteringen door te voeren?
