Functieomschrijving Als POH-GGZ ben je werkzaam binnen een huisartsenpraktijk. Je ondersteunt de huisarts bij het verlenen van zorg aan patiënten met psychische problemen. Het doel is de geestelijke gezondheidszorg binnen de huisartsenpraktijk te versterken en een bijdrage te leveren aan adequate goede samenwerking met andere GZ-zorgverleners. Wat ga je doen? - Ondersteuning van de huisarts middels het zien van de patiënten met GGZ problematiek - Uitvoeren van probleemverduidelijking en vraagverheldering - Behandeling verlenen onder verantwoording van de huisarts - Casemanagement - Begeleiden van chronisch psychiatrische patiënten - Opbouwen en onderhouden van je sociale kaart en netwerk - Inzetten en begeleiden van e-health modules - Wachtlijstenoverbrugging Wie ben jij? Als POH-GGZ beschik je over goede sociale vaardigheden. Met jouw betrokkenheid en sterke communicatieve vaardigheden weet je patiënten te stimuleren en motiveren. Omdat de patiëntengroep in de huisartsenpraktijk generalistisch is, krijg je patiënten in verschillende leeftijdscategorieën. Wij verwachten niet dat je ervaring hebt met alle doelgroepen maar je moet wel openstaan om iedereen te begeleiden. Het is ook belangrijk dat je kan reflecteren op je eigen handelen en dat je openstaat voor feedback tijdens intervisie en scholing.
