Praktijkondersteuner (POH) Utrecht - Terwijde 24-27 uur per week Ben jij een gediplomeerd en betrokken POH die graag écht de tijd neemt voor patiënten? Wil je werken in een multidisciplinair gezondheidscentrum waar samenwerking, persoonlijke aandacht en preventieve zorg centraal staan? Dan zijn wij op zoek naar jou! Er is ruimte om te overleggen en van elkaar te leren. Zo neem je actief deel aan overleggen met praktijkondersteuners van de andere locaties van Leidsche Rijn Julius Gezondheidscentra. Daarnaast kun je meedoen aan innovatie en onderzoek in samenwerking met onder meer het UMC Utrecht en de Hogeschool Utrecht. Geïnteresseerd? Team HR [email protected] Solliciteer direct Als POH bij Leische Rijn Julius Gezondheidscentra - begeleid je patiënten met chronische of complexe gezondheidsproblemen zoals diabetes, COPD/astma en CVRM; - geef je patiënten voorlichting, advies en coaching om hun zelfmanagement te versterken en de regie over hun eigen gezondheid te houden; - draai je zelfstandig spreekuur, coördineer je de zorg en adviseer je over medicatiegebruik en leefstijl; - draag je bij aan kwaliteit, protocollen en verbetering van de zorg; - is er ruimte om mee te doen aan innovaties en onderzoeken. Wat vragen we van jou? - Je bent een gediplomeerd praktijkondersteuner/-verpleegkundige. - Je hebt bij voorkeur ervaring met het begeleiden van patiënten met een chronische aandoeningen en/of ouderenzorg. - Je werkt graag samen, bent communicatief sterk en weet collega's te vinden. - Je hebt interesse in het werken binnen de dynamische omgeving van een groot ...
