Praktijkondersteuner Somatiek Berkel en Rodenrijs Fulltime, Parttime 26 - 38 uur Overig 16-10-2026 Zorg / Welzijn Functie Gezondheidscentrum Berkel en Rodenrijs ligt in de VINEX-wijk Meerpolder van de gemeente Lansingerland. Werken bij gezondheidscentrum Berkel en Rodenrijs betekent dat je deel uitmaakt van een gemotiveerd team van huisartsen, assistentes, praktijkondersteuners en een verpleegkundig specialist. Ons patiëntenbestand bestaat uit ongeveer 9.000 inschrijvingen gekenmerkt door een jonge populatie. In de praktijk heerst een open en informele teamgeest. Er is een goed georganiseerd samenwerkingsverband op het gebied van de eerstelijnsgezondheidszorg. Zo werken wij samen met de andere deelnemers van het gezondheidscentrum zoals een diëtiste, fysiotherapeuten, logopedie, centrum voor jeugd en gezin en een apotheek. In het pand bevindt zich ook een buitenpolikliniek van het Franciscus Gasthuis. Wat ga je doen? Als Praktijkondersteuner Somatiek ondersteun je de huisartsen bij het verlenen van laagdrempelige zorg. In deze functie stellen wij bijzondere eisen aan zelfstandig kunnen werken. Je taken bestaan vooral uit consultatie, kortdurende behandeling, probleemverheldering, administratie bijhouden, huisbezoeken uitvoeren (indien nodig), medische behandelingen uitvoeren, diagnostiek en zo nodig doorverwijzing. Daarnaast geef je ook voorlichting over ziektebeelden, medicatie en preventie. Wij vragen Wie zoeken wij? - Een geregistreerde POH-S bij voorkeur met ervaring in de huisartsenpraktijk; - Een betrokken professional met affiniteit voor de eerstelijnszorg en een proactieve houding; - Communicatief sterk, motiverend en vaardig in het begeleiden van patiënten bij leefstijl- en gedragsverandering; - Iemand die zelfstandig werkt en graag samenwerkt binnen een multidisciplinair team; - Beschikbaar voor 25,5-38 uur per week, bij voorkeur met inzet op maandag.
