Over onze praktijk Huisartsen de Burgt is een moderne, toegankelijke en warme praktijk waar kwaliteit van zorg en persoonlijke aandacht centraal staan. Je komt terecht in een groot en collegiaal team. Samen zorgen wij voor een breed en veelzijdig zorgaanbod voor onze patiënten. De functie Als praktijkverpleegkundige/POH-S werk je zelfstandig en in samenwerking met collega's binnen verschillende zorgprogramma's. Je houdt je onder andere bezig met het voeren van zelfstandig spreekuren betreffende: - COPD/Astma, inclusief longfunctiemetingen - Diabetes Mellitus, inclusief begeleiding bij insulinetherapie - CVRM/Atriumfibrilleren - SMR - Ouderenzorg Wat bieden wij? - Een zelfstandige, afwisselende en uitdagende functie binnen een betrokken team - Een prettige werksfeer met oog voor samenwerking en ontwikkeling - Arbeidsvoorwaarden en salaris volgens de cao Huisartsenzorg - De mogelijkheid om te groeien en je verder te specialiseren Wat neem jij mee?
