Procesregisseur Sociaal Domein Ben jij die verbindende factor die overzicht houdt wanneer een casus vastloopt en meerdere partijen om tafel moeten? Als procesregisseur in het sociaal domein breng jij structuur en regie aan in complexe, vaak meervoudige vraagstukken. Je zorgt dat professionals hun verantwoordelijkheid nemen, dat er daadwerkelijk wordt gehandeld en dat gezinnen en inwoners de hulp krijgen die nodig is. In deze functie coördineer je complexe casussen waarin meerdere levensgebieden en partijen een rol spelen. Je organiseert en zit multidisciplinaire overleggen voor, bewaakt de voortgang van het plan van aanpak en zorgt dat gemaakte afspraken worden nagekomen. Je werkt nauw samen met onder meer wijkteams, jeugdhulpaanbieders, gemeenten en veiligheidspartners, en je bent het aanspreekpunt zodra een casus dreigt vast te lopen. Je taken bestaan uit: - Coördineren en regisseren van complexe casussen binnen het sociaal domein; - Organiseren en voorzitten van multidisciplinaire overleggen; - Bewaken van voortgang, kwaliteit en afspraken binnen het plan van aanpak; - Verbinden van ketenpartners en hen aanspreken op hun rol en verantwoordelijkheid; - Signaleren van trends en knelpunten en deze vertalen naar verbetervoorstellen; - Adviseren over passende interventies en vervolgstappen. Je maakt deel uit van een betrokken team waarin samenwerking, korte lijnen en persoonlijke ontwikkeling centraal staan. Er is ruimte voor coaching, intervisie en verdere professionalisering, binnen een organisatie waar de mens centraal staat en jij de vrijheid krijgt om je eigen aanpak vorm te geven. - HBO werk- en denkniveau, opleiding gericht op het sociaal domein of vergelijkbaar - SKJ-registratie - Minimaal 1 jaar relevante werkervaring binnen het sociaal domein - Sterke communicatieve en coördinerende vaardigheden - Overzicht houden en besluitvaardig handelen in complexe, soms gespannen situaties - Beschikbaar voor 28 - 36 uur per week ...
