Je organiseert, coördineert en begeleidt implementatiestappen, signaleert knelpunten en bereidt borging praktisch voor zodat toepassing duurzaam kan worden voortgezet. Taken en verantwoordelijkheden - Organiseren, coördineren en begeleiden van de implementatie van AVE binnen gemeentelijke teams. - Breed uitrollen van de Meldcode binnen de organisatie. - Ondersteunen van teams bij toepassing van AVE en Meldcode (bijeenkomsten, uitleg, materialen). - Afstemmen met interne teams en waar nodig externe partners. - Structuur aanbrengen of aansluiten op bestaande structuren (bijv. verbinding tussen zorg & veiligheid) voor evaluatie, monitoring en borging, zodat deze duurzaam voortgezet kunnen worden. - Signaleren van knelpunten en terugkoppelen richting beleid en management. - Verzorgen van (tussen)rapportages, verslagen en terugkoppeling richting opdrachtgever. Kandidaatprofiel - Ervaring: Aantoonbare ervaring als projectleider, bij voorkeur binnen een gemeentelijke organisatie. - Implementatie: Praktische ervaring met implementatie van projecten, bij voorkeur op het gebied van zorg, veiligheid of sociale interventies. - Netwerkkennis: Kennis van netwerkpartners op het gebied van GIA/OGGz/Wvggz/Wzd. - Pré: Kennis of affiniteit met AVE en/of Meldcode is een pré. Vaardigheden en competenties - Samenwerken en afstemmen: Je kunt goed schakelen tussen interne teams en waar nodig externe partners. - Informatie & terugkoppeling: Je weet wie wanneer geïnformeerd moet worden en verzorgt heldere rapportages. - Competenties: Zelfstandig, proactief, communicatief sterk, resultaatgericht en organisatorisch vaardig.
