RegiozorgNU is op zoek naar een aanjager palliatieve zorg die tot en met 31 december 2027 werk maakt van de implementatie van de transformatie palliatieve zorg in de huisartsenpraktijken in Noordwest- Utrecht. RegiozorgNU is dé regionale organisatie voor huisartsenzorg in Noordwest-Utrecht. Samen verbeteren en vernieuwen we de eerstelijnszorg, spoedzorg en chronische zorg. Zodat inwoners van Noordwest-Utrecht ook in de toekomst verzekerd zijn van goede zorg en zorgverleners hun werk met plezier kunnen blijven doen. Wat ga je doen? We werken samen met 35 partijen aan de transformatie van de palliatieve zorg. Als projectleider zorg jij ervoor dat huisartsenpraktijken in onze regio op de hoogte zijn van de nieuwe werkafspraken en de nieuwe tools en mogelijkheden actief benutten. Samen met collega's uit onder andere de wijkverpleging en ziekenhuizen werk je aan een gezamenlijke beweging, zodat inwoners in onze regio kunnen rekenen op passende en goed afgestemde palliatieve zorg. Je bent projectleider van een klein multidisciplinair projectteam binnen RegiozorgNU met een huisarts, communicatie deskundigheid en projectleider digitalisering. Jij houdt het overzicht, zorg voor een duidelijke koers, speelt in op actuele ontwikkelingen en zorgt ervoor dat jullie samen passende interventies ontwikkelen en uitvoeren, binnen de zes projectlijnen. Je bent verantwoordelijk voor de transformatie binnen de huisartsenzorg. Dat doe je natuurlijk niet alleen: de verandering komt tot stand in samenwerking met de verschillende professionals en organisaties die betrokken zijn bij de transformatie.
