Bij het team Intensive Home Treatment (IHT) in Zwolle hebben wij ruimte voor een sociaal psychiatrisch verpleegkundige om ons team te versterken. Wat ga je doen? Als (senior) psychiatrisch verpleegkundige bied je direct behandeling aan patiënten die in crisis verkeren. Je werkt zoveel mogelijk in de eigen leefomgeving van de cliënt en betrekt actief naastbetrokkenen bij de behandeling. Samen met collega's help je crisissen te stabiliseren, herstel te bevorderen en waar mogelijk een opname te voorkomen. Het participeren in groepstrainingen en/of -behandelingen maakt deel uit van je takenpakket. De werkzaamheden worden door de weeks in wisselende diensten tussen 8.30-22.00 uitgevoerd. In het weekend is dit van 08:30 t/m 17:00 uur. Het team IHT Zwolle is onderdeel van de Spoedeisende GGZ en bestaat uit een multidisciplinair team dat zich richt op crisisinterventie, intensieve thuisbehandeling en het voorkomen van klinische opnames. Het team werkt vanuit de visie dat mensen zoveel mogelijk in hun eigen omgeving behandeld worden. De zorg kan flexibel worden op- en afgeschaald, afhankelijk van wat de cliënt nodig heeft. Hierdoor kunnen opnames vaak worden voorkomen of zo kort mogelijk gehouden worden. Binnen het team werken we met een gedeelde caseload en ondersteunen we elkaar waar nodig.
