We hebben ruimte voor 24 tot 36 uur per week. Je maakt deel uit van een multidisciplinair team, waarbij er ruimte is om door te groeien via een opleidingstraject tot sociaal psychiatrisch verpleegkundige (SPV)! Wil jij teamoverstijgend werken als psychiatrisch verpleegkundige binnen de (jong) volwassenenzorg? Wij zoeken iemand die het leuk vindt om teamoverstijgend werken tussen de teams MET Focus (basis ggz), MET Persoonlijkheid, MET Psychotrauma, MET ADHD, MET ADS (Angst, Dwang en Stemming), Ouderenzorg en Jongvolwassenen. Deze gespecialiseerde teams werken nauw met elkaar samenwerken. Wat houdt de functie in? Als Algemeen Psychiatrisch Verpleegkundige (APV) bij MET ggz houd jij je bezig met: - Het vertalen van het behandelplan naar concrete doelen en activiteiten, gericht op onder andere activering, structurering en het versterken van zelfmanagement. - Het ondersteunen van cliënten bij zowel medisch als persoonlijk herstel. - Het signaleren van psychopathologie en somatische aandoeningen in relatie tot het psychisch functioneren van de cliënt. - Het uitvoeren van verpleegkundige diagnostiek. - Het begeleiden en uitvoeren van behandelonderdelen, zowel in de thuissituatie als eventueel in groepsverband, passend binnen het behandelplan. - Het afstemmen van zorg met interne en externe hulpverleners. - Het geven van voorlichting en advies aan cliënten, naasten en hun netwerk. - Het bewaken van de effecten van medicatie die is voorgeschreven door de behandelend arts of verpleegkundig specialist GGZ.
