Wij bieden geïntegreerde zorg aan mensen met een ernstige psychiatrische aandoening, vanuit een samenhangend pakket van wonen, behandeling en dagbesteding. De betrokkenheid van familie vormt een kernwaarde. Met 15 woonlocaties en diverse dagbestedingsprojecten bouwen we dagelijks aan een omgeving waar cliënten zich gezien, gehoord en gewaardeerd voelen. Jouw werkplek en werkzaamheden Als psychiatrisch verpleegkundige binnen het FACT-team bied je herstelondersteunende zorg aan cliënten met ernstige psychiatrische aandoeningen. Je werkt outreachend en bezoekt cliënten in hun eigen leefomgeving, op woonlocaties of op kantoor. Je bouwt een professionele behandelrelatie op en ondersteunt cliënten bij het vergroten van hun zelfstandigheid, stabiliteit en kwaliteit van leven. Je signaleert veranderingen in het psychiatrisch en somatisch functioneren, voert verpleegkundige interventies uit en draagt bij aan vroegsignalering, crisispreventie en een verantwoord medicatiebeleid. Je bent dossierhouder en hebt een eigen caseload. Samen met collega's uit het multidisciplinaire FACT-team stem je dagelijks de behandeling af en werk je aan uitvoering en evaluatie van behandelplannen. Je onderhoudt contact met naasten en ketenpartners en levert een actieve bijdrage aan de continuïteit van zorg. Na een inwerkperiode neem je deel aan de achterwachtdiensten. Jij bent iemand die: - In het bezit is van een afgeronde hbo-opleiding verpleegkunde. - Ervaring heeft (of affiniteit) met het begeleiden van mensen met een psychische kwetsbaarheid. - Kennis heeft van psychopathologie, psychofarmaca en diverse evidence-based methodieke...
