Kompas vzw biedt zorg op maat aan personen en hun context die problemen ervaren door het misbruik van middelen en/of verslavend gedrag vertonen. We bieden ondersteuning en begeleiding van zorgvragers en hun context, van laagdrempelige zorg tot residentiële behandeling met als doel herstel.

Subsidiariteit, diversiteit, emancipatorisch werken en persoonsgerichte zorg zijn als organisatie centrale normen en waarden binnen ons behandelaanbod.

* Gerechtelijke Alternatieve Maatregelen, Ieper (50%)

Binnen GAM begeleid je cliënten die vanuit justitie worden doorverwezen in het kader van een gerechtelijke maatregel. De focus van de begeleiding ligt op het middelengebruik en de invloed ervan op verschillend levensdomeinen. Je werkt onder meer rond motivatie, inzicht, hervalpreventie en het voorkomen van nieuw grensoverschrijdend gedrag. Daarbij hou je rekening met zowel de hulpvraag van de cliënt als het justitiële kader waarbinnen de begeleiding plaatsvindt. De GAM-werking van Kompas zet in op forensische hulpverlening, met aandacht voor modellen en methodieken zoals hervalpreventie, delictscenario’s en het Good Lives Model.

* Zorgtraject Geïnterneerden, Kortrijk (50%)

Het Zorgtraject Geïnterneerden is een samenwerkingsproject tussen het Centrum voor Geestelijke Gezondheidszorg Mandel en Leie, Kompas vzw, saMENSpel, Eigen Woonst en het Psychiatrisch Centrum Menen. Samen bundelen deze organisaties hun krachten om kwalitatieve en geïntegreerde zorg te bieden aan mensen met een interneringsstatuut. Vanuit een netwerkgerichte en vraaggestuurde aanpak wordt voor elke cliënt een zorgpad op maat uitgewerkt. Daarbij is er zowel aandacht voor de noden en hersteldoelen van de cliënt als voor de professionele inzichten van de betrokken hulpverlening. De samenwerking tussen ambulante en residentiële partners ondersteunt een vlotte doorstroom tussen verschillende zorgvormen. Tijdens interdisciplinair casusoverleg wordt gezamenlijk gezocht naar passende, duurzame ondersteuning, met aandacht voor herstel, continuïteit van zorg en maximale maatschappelijke integratie. Het Zorgtraject Geïnterneerden wil bijdragen aan een vlotte uitstroom van geïnterneerde personen uit gevangenissen, Forensisch Psychiatrische Centra en andere residentiële voorzieningen met interneringsplaatsen naar een gepast zorgcircuit. Binnen dit circuit wordt een individueel zorgpad uitgewerkt en opgevolgd.

* voert intake- en begeleidingsgesprekken met cliënten; brengt samen met de cliënt het middelengebruik en de verschillende relevante levensdomeinen in kaart; werkt herstelgericht aan motivatie, gedragsverandering en het versterken van beschermende factoren; ondersteunt cliënten bij het voorkomen van herval en recidive;

* maakt samen met de cliënt duidelijke en haalbare begeleidingsdoelstellingen op; stemt af met justitie-assistenten, verwijzers en andere betrokken hulpverleners; bewaakt binnen de begeleiding het evenwicht tussen zorg, maatschappelijke veiligheid en het opgelegde justitiële kader; zorgt voor een correcte dossiervorming, verslaggeving en terugkoppeling.

* begeleidt personen met een interneringsstatuut bij de uitbouw en opvolging van hun individuele zorgtraject; ondersteunt cliënten bij hun herstel, maatschappelijke integratie en doorstroom naar passende zorg; voert psychologische gesprekken en klinische inschattingen uit; neemt actief deel aan interdisciplinair casusoverleg
