Als referentieverpleegkundige ben jij de draaischijf van expertise rond diabetes in onze Thuisverpleging. Je combineert je dagdagelijkse zorgtaken met een rol als inhoudelijk expert. Zo ben je een aanspreekpunt voor je collega’s, een klankbord voor de klanten, familie en mantelzorgers en een inspiratiebron om onze kwaliteit te verhogen.

Je krijgt de ruimte om je inhoudelijk te verdiepen en die kennis actief te delen met al je collega’s binnen de thuisverpleging. Je begeleidt en ondersteunt collega’s, werkt mee aan zorgvernieuwing en aan projecten binnen jouw vakgebied. Zo zorg jij ervoor dat we in onze thuisverpleging niet enkel goede zorg bieden, maar excellente zorg.

Jouw impact:

  • Je neemt het voortouw in jouw referentiedomein diabetes.
  • Je ondersteunt collega’s met advies, vorming en coaching op de werkvloer.
  • Je werkt mee aan zorginhoudelijke projecten en kwaliteitsverbetering.
  • Je bent een aanspreekpunt bij complexe zorgvragen.
  • Je combineert je referentierol met je dagelijkse zorgtaken binnen de thuisverpleging.
  • Je hebt een diploma als verpleegkundige + je bent in bezit van een bijzondere beroepsbekwaamheid (BBK) Diabetologie door het slagen in een Postgraduaat Diabeteseducator.
  • Je bent een teamspeler met een gezonde dosis initiatief.
  • Je communiceert vlot en bent gepassioneerd door diabetes.
