Een ander probeert na een psychose het dagelijks leven weer op te pakken. En soms weet iemand zelf even niet meer hoe de volgende stap eruit moet zien. Als regieverpleegkundige kijk jij verder dan de diagnose. Je brengt rust, overzicht en structuur en zoekt samen met de patiënt naar wat wél mogelijk is. In deze functie begeleid je volwassenen met uiteenlopende psychiatrische problematiek, zoals angst- en stemmingsstoornissen, trauma, persoonlijkheidsproblematiek en psychotische aandoeningen. Je combineert directe patiëntenzorg met verpleegkundig leiderschap. Je houdt de regie over het verpleegkundige zorgproces en werkt nauw samen met behandelaren, naasten en ketenpartners. Je wordt onderdeel van een enthousiast en gedreven gebiedsteam in Almere Noord en Midden.Samen bieden jullie herstelgerichte en outreachende zorg. Je werkt veel samen met collega's, wijkteams, behandelaren en andere ketenpartners. Het team is volop in ontwikkeling. Er wordt hard gewerkt, maar er is ook ruimte voor humor, initiatief en plezier. Je krijgt verantwoordelijkheid, maar staat er nooit alleen voor. Wat je gaat doen - Je voert regie over het verpleegkundige zorgproces en bewaakt de voortgang. - Je begeleidt patiënten vanuit een herstelgerichte en persoonlijke benadering. - Je coacht collega's en stimuleert verpleegkundig leiderschap binnen het team. - Je initieert verbeteringen rondom kwaliteit, digitalisering en werkprocessen. - Je past klinisch redeneren en Evidence Based Practice toe en deelt jouw kennis.
