Je legt contact, bouwt vertrouwen op en brengt kansen, ondersteuningsbehoeften en belemmeringen in kaart. Je kijkt verder dan werk alleen en helpt inwoners stappen te zetten richting participatie en maatschappelijke deelname. Wat ga je doen? - Het voeren van gesprekken met inwoners, zowel op kantoor als tijdens huisbezoeken - Het opbouwen van een vertrouwensband met inwoners - Het in kaart brengen van de situatie en ondersteuningsbehoeften van inwoners op verschillende leefgebieden - Het signaleren van kansen, belemmeringen en mogelijkheden voor ontwikkeling en participatie - Het stimuleren en begeleiden van inwoners bij het zetten van passende vervolgstappen - Het organiseren of in gang zetten van passende ondersteuning - Het onderhouden van contacten met samenwerkingspartners binnen en buiten de gemeente - Het afstemmen van bevindingen en vervolgstappen met de betrokken klantmanager - Het zorgvuldig vastleggen van gesprekken, acties en resultaten - Het bijdragen aan een actueel en compleet beeld van de doelgroep binnen het project Wie zoeken we? Je maakt gemakkelijk contact, bent oprecht geïnteresseerd in mensen en hebt het geduld om samen met inwoners kleine maar betekenisvolle stappen te zetten. Je begrijpt dat ontwikkeling tijd kost en weet vertrouwen op te bouwen bij mensen die soms al langere tijd weinig perspectief hebben ervaren. Vereisten en gewenste kwaliteiten: - Hbo werk- en denkniveau - Ervaring als werkcoach, participatiecoach, klantmanager, consulent of in een vergelijkbare functie binnen het sociaal domein - Affiniteit met inwoners die een grote afstand hebben tot participatie - Sterk in het voeren van motiverende gesprekken - Vermogen om verbinding te leggen tussen inwoners, collega...
