BlueTrail Regievoerder Sociale Inclusie Groningen 32-36u p/w 6 maanden HBO Functieomschrijving Je gaat aan de slag bij Gemeente Oldambt binnen het project Sociale Inclusie. Dit project richt zich op inwoners met een bijstandsuitkering die een grote afstand hebben tot participatie en voor wie betaald werk niet altijd direct haalbaar of passend is. Je werkt binnen een klein en betrokken projectteam en stemt regelmatig af met klantmanagers, collega's binnen het sociaal domein, samenwerkingspartners en andere betrokken professionals. De opdracht Als Regievoerder Sociale Inclusie ben jij de schakel tussen de inwoner, de klantmanager en het netwerk rondom de inwoner. Je legt contact, bouwt vertrouwen op en brengt de situatie, ondersteuningsbehoeften, kansen en belemmeringen van inwoners in kaart. Daarbij kijk je verder dan werk alleen en heb je oog voor verschillende leefgebieden. Door gesprekken, huisbezoeken en afstemming met betrokken professionals onderzoek je samen met de inwoner welke stappen passend en haalbaar zijn. Dit kan bijvoorbeeld vrijwilligerswerk, dagbesteding, sociale activiteiten of het versterken van het netwerk zijn. Soms is eerst ondersteuning op andere leefgebieden nodig. Jij helpt inwoners om weer in beweging te komen en naar vermogen mee te doen in de samenleving. Wat ga je doen? - Je voert gesprekken met inwoners op kantoor en tijdens huisbezoeken. - Je bouwt een vertrouwensband op en brengt de situatie en ondersteuningsbehoeften in kaart. - Je signaleert kansen, belemmeringen en mogelijkheden voor ontwikkeling en participatie. - Je stimuleert en begeleidt inwoners bij passende vervolgstappen. - Je organiseert of zet passende ondersteuning in gang. - Je onderhoudt contact met samenwerkingspartners binnen en buiten de gemeente. - Je stemt bevindingen en vervolgstappen af met de betrokken klantmanager. - Je legt gesprekken, acties en resultaten zorgvuldig vast.
