" Ben jij op zoek naar een zelfstandige functie waarin jij samen met een collega regisserend wijkverpleegkundige jullie team coacht? Daarnaast werk je nauw samen met de collega's uit de nabij gelegen teams. Wij zoeken een regisserend wijkverpleegkundige voor ons team in Delfzijl. Waarbij er de mogelijkheid is te groeien in de functie. Word jij onze collega? Over deze leuke baan Samen met het team werk je nauw samen met als doel de best mogelijke zorg te verlenen, met oog voor de mens achter de zorgvraag. Als regisserend wijkverpleegkundige beschik je over uitstekende communicatieve vaardigheden en een duidelijke visie op de wijkverpleging. Je houdt verbinding met de cliënt en de collega's door zelf mee te draaien op routes. De afwisseling tussen coördineren, regisseren, indiceren én uitvoeren maakt deze functie juist zo uitdagend en afwisselend. Na de route ben je aanwezig op het wijkkantoor voor jouw team om ervaringen of bijzonderheden te delen. Waar nodig schakel je ook met andere zorgverleners, zoals huisartsen en ziekenhuizen. Elke cliënt is anders en geen dag is hetzelfde. Daarnaast kom je elke zes weken met het hele team samen voor een groot overleg; voorafgaand daaraan gaan jullie eerst gezellig lunchen om bij te praten....
