Je neemt als ggz-agoog in de rol van rehabilitatiemedewerker de participatiewensen van jouw cliënten als uitgangspunt en ondersteunt bij hun herstelproces. Je ondersteunt de cliënten bij het kiezen, herwinnen en behouden van hun gewenste kwaliteit van leven op de verschillende levensgebieden, waaronder; gezondheid, fysiek en geestelijk, werk, sociale contacten, wonen financiën, persoonlijke hygiëne en vrije tijd. Met het team Je zal werken binnen team Persoonlijkheid en team Psychotrauma. Team Persoonlijkheidsstoornissen is een multidisciplinair team in Maastricht, dat specialistische/multidisciplinaire GGZ hulp biedt aan mensen met ernstige persoonlijkheidsproblematieken en persoonlijkheidsstoornissen. Er wordt gewerkt volgens het principe " groepen tenzij" , waarbij in principe alle cliënten groepsbehandeling krijgen, met daarnaast individuele sessies waar nodig of conform de protocollen. De behandelaren werken met het GIT-PD (Guideline Informed Treatment for Personality Disorders) kader, wat ons als team structuur geeft om met cliënten een effectieve behandelroute en relatie te ontwikkelen. Na afronding van de GIT -PD fase, stromen cliënten door naar specifieke behandelprogramma's bijvoorbeeld Schema Focused Therapy (SFT), Mentalization Based Treatment (MBT) of een VERS behandeling. Parallel kan er medicamenteuze behandeling, vaktherapie, rehabilitatie of IPS ingezet worden. Hierdoor kunnen we de juiste zorg bieden.
