Over de functie Als senior projectleider ben je verantwoordelijk voor het opzetten en uitvoeren van de Proeftuin Toegang in een buurt in Ede. Je werkt samen met inwoners, professionals, maatschappelijke partners en de gemeente aan een integrale en laagdrempelige toegang tot zorg en ondersteuning. Je ontwikkelt, test en verbetert de nieuwe werkwijze in de praktijk en levert concrete plannen, evaluaties en aanbevelingen op voor verdere ontwikkeling. Ook adviseer je het programma Hart voor HEZ over de toegang in Ede Zuid. Wat je gaat doen - Vaststellen van doelen, aanpak en succescriteria van de proeftuin met de opdrachtgever - Opstellen van een plan van aanpak inclusief planning, begroting, rolverdeling en monitoring - Inrichten, uitvoeren en bewaken van de voortgang van de proeftuin - Ontwikkelen van de nieuwe toegang samen met inwoners, professionals en partners - Verbinden van toegang, sociale basis en lokale teams - Organiseren van samenwerking met wijkpartners - Maken van afspraken met gemeentelijke afdelingen - Uitvoeren van opdrachten aan betrokken partners - Monitoren van voortgang en organiseren van tussenevaluaties - Afstemmen met het deelproject buurtopgave en zorgen voor samenhang - Vastleggen van ervaringen en leerpunten en vertalen naar aanbevelingen Functie-eisen - HBO opleidingsniveau - Minimaal 5 jaar aantoonbare ervaring als senior projectleider binnen het sociaal domein of vergelijkbare publieke context - Aantoonbare ervaring met het organiseren of vernieuwen van de toegang tot zorg en ondersteuning binnen het sociaal domein - Aantoonbare ervaring met wijkgericht werken en samenwerking met inwoners, inform...
