Erasmus MC · Rotterdam Functie inhoud Als (senior) verpleegkundig consulent werk je in een enthousiast, hecht en deskundig team, waarin collega's nauw samenwerken om de beste zorg te bieden. Je bent voor patiënten en hun naasten een vast aanspreekpunt gedurende het behandeltraject. Je begeleidt patiënten en draagt bij aan (hoogwaardige), persoonsgerichte zorg. Je houdt je onder andere bezig met: - Coördineren en organiseren van zorgtrajecten. - Geven van voorlichting, begeleiding en coaching aan patiënten en hun naasten. - Signaleren van lichamelijke, psychosociale en maatschappelijke problematiek. Daarnaast ben je de verbindende factor tussen patiënt, artsen, verpleegkundigen en andere disciplines. Je neemt deel aan multidisciplinaire overleggen en draagt kennis over aan collega's binnen het team en samenwerkende zorgprofessionals. Werkomgeving De afdeling Longgeneeskunde vormt samen met Cardiologie en Thoraxchirurgie het Thoraxcentrum. Binnen de Longgeneeskunde wordt academische zorg geleverd aan onder andere patiënten met Cystic Fibrosis, longinfectieziekten en congenitale longziekten. Voor Cystic Fibrosis en aangeboren misvormingen van de long is de afdeling erkend als expertisecentrum door VWS. De werkomgeving is dynamisch en inhoudelijk sterk: kwaliteit staat centraal en onderzoek wordt ondersteund door een grote researchafdeling. Je komt terecht in het team Cystic Fibrosis, Infectie en Congenitale Longziekten. Dit team bestaat uit longartsen, verpleegkundig consulenten en een verpleegkundig specialist.
